Select All Residents with one Click and select Flu Shot Consent form and click the ‘Send’ button. Each POA for every resident will get an email link to sign the consent form online.

As each POA signs the form, the counts of completed forms are displayed on your screen and each consent form is AUTOMATICALLY filed in the appropriate resident’s chart.

For unsigned (late) forms, click on the ‘Remind’ button to send another email reminder.

Once forms are signed, you instantly receive them, and they are automatically filed in the resident/client’s chart.

If you need to send it to the pharmacy, select multiple signed forms and click on ‘Send Fax’ to send all the forms by electronic fax to the Pharmacy.
